The bar is derived from these five facts, not set by hand. Human study counts follow the weekly PubMed refresh; the rest change only when a trial is published or a regulator acts.
How strong the effect is
strength 2 of 5 · weight 0.35
Noticeable but modest
Cohort data from a single institute.
What's unique
Almost the entire evidence base comes from one Saint Petersburg institute over three decades.
Unexpected positive evidence
A subset of the cohort work reported melatonin normalisation before any telomere claim.

Benefits

telomerase activation and support for telomere length in dividing cells
Cellular maintenance · from week 4
restoration of melatonin rhythm and pineal signalling with age
Sleep · from week 2
normalisation of cortisol and gonadotropin cycles disturbed by ageing
Cellular maintenance · from week 4
antioxidant protection and reduced age-related lipid peroxidation
Cellular maintenance · from week 4
immune reconstitution through pineal–thymic cross-talk
Cellular maintenance · from week 4
support for healthy ageing that accumulates over months, not days
Healthspan · from week 10

Timeline

Days 1–10
The classic course length. Sleep timing shifts first: falling asleep earlier, waking at a consistent hour, fewer 3am wake-ups. Little else is noticeable.
Weeks 2–4
Sleep depth consolidates and daytime energy follows it. Most of what people describe as an "anti-ageing" effect at this stage is simply sleeping properly again.
Months 2–6
The window the Russian cohort work measured. Any claimed effect on ageing markers sits here and is not something you can feel. Judge this stage by sleep and recovery, not by promises.
Post-cycle
Sleep timing drifts back over about a month. The standard pattern is two to three short courses a year rather than continuous use.

Risks

Short-term
Sleep-pattern shifts in the first week.
Long-term
Telomerase activation and cancer risk are theoretically linked. Nobody has studied this in humans.
Tolerance & withdrawal
Run in 10–20 day cycles, twice a year, by convention rather than evidence.

Dosing

Dose
5–10 mg per injection
10 mg is the course dose in almost every published protocol.
Route
Subcutaneous — abdomen
Any belly site works. The dose is small enough that technique barely matters.
Timing
Evening, near the melatonin window
Evening, to sit near the melatonin window it is supposed to influence.
Frequency
Daily for the length of the course
Daily is the only pattern used — the course is short enough not to need a pause.
Cycle
10–20 days on, then months off
Ten to twenty days, then nothing for months. Two or three courses a year.
Duration
Short courses
The whole design is a short pulse repeated rarely — running it monthly is not how any protocol was written.

Forms

injectable
~100%
+ Full dose delivered; nothing lost to first-pass
− Daily needles, sites, sharps disposal
Refrigerate 2–8 °C after reconstitution
nasal
Product-specific
+ A needle-free route listed in the source catalog
− Absorption and safety cannot be inferred from the injectable form
Follow the specific product storage instructions

Nasal forms are listed in the source catalog; this does not establish approval, efficacy or equivalent absorption. HEIVE injection calculations do not apply to nasal forms.
